FOR REFERRING PROVIDERS

COLLABORATIVE PSYCHOLOGICAL CARE for Your Patients

Evidence-based psychological care for complex clinical, behavioral, and functional concerns.

Commonwealth Center for Behavioral Medicine provides specialized psychological care for adolescents and adults across mental health, behavioral medicine, health psychology, and performance. We work collaboratively with physicians, psychiatrists, therapists, schools, and other professionals to address concerns that may extend beyond traditional clinical boundaries.

WHEN CCBM MAY BE A GOOD REFERRAL

CCBM May Be Particularly Well Suited For Individuals Who:

  • Have concerns that cross traditional clinical boundaries, such as mental health, health behavior, sleep, and performance

  • Are experiencing multiple interacting psychological, behavioral, or health-related concerns

  • Have not obtained sufficient benefit from previous treatment or need a different clinical approach

  • Need an evidence-based, structured approach to treatment

  • Would benefit from a structured, goal-oriented approach to care

  • Are navigating significant developmental or functional transitions

  • Would benefit from coordination among psychological and medical providers

A COLLABORATIVE APPROACH

Working Alongside the Existing Care Team

CCBM does not require patients to transfer existing psychiatric or medical care to the practice. CCBM is designed to complement, rather than replace, the care patients are already receiving.

With appropriate authorization, CCBM can coordinate with referring and treating professionals, including:

  • Primary care physicians

  • Psychiatrists and psychiatric nurse practitioners

  • Other mental health clinicians

  • Pediatricians and other medical specialists

  • Schools and educational professionals

  • Other professionals involved in a patient's care

Coordination may include sharing relevant clinical information, discussing treatment recommendations, and communicating about progress or changes in clinical needs.

We review the referral information to determine whether CCBM appears to be an appropriate setting and level of care.

03

Consultation & Assessment

When appropriate, the prospective client completes an initial consultation and clinical assessment to clarify treatment needs and goals.

Treatment & Care Coordination

If treatment is initiated, CCBM provides individualized evidence-based care and coordinates with other members of the treatment team when appropriate and authorized.
